Project Background

In response to the increasing number of elderly singletons and doubletons experiencing social isolation, The Hong Kong Jockey Club Charities Trust donated about $380 million to implement the Jockey Club "Reach to Bridge" Community Elderly Support Project (“the Project”). Launched in 2023, the Project has successfully expanded to eighteen districts, demonstrating a strong commitment to enhancing community support and fostering social connections among elderly singletons and doubletons.

Since its inception, the Project has proactively reached out to isolated elderly through non-governmental organisations (“NGOs”) and local organisations. With support from The University of Hong Kong in developing the needs assessment protocol, project partners can evaluate the service requirements of the elderly, enabling them to provide timely assistance and appropriate referrals tailored to individual needs. Furthermore, the Project also provides home improvements for eligible households, facilitating them to age in place within a safe and accessible environment.

Target beneficiaries

  • Aged 60 or above
  • Singleton or doubleton elderly1
  • Lack of family support2 and not receiving any government-subsidised elderly services in the past 12 months3

Engage hard-to-reach elderly

To proactively reach out and reconnect socially isolated singleton or doubleton elderly through an array of programmes, such as roadshow, home visit, and district-based activities, etc.

Assess service needs and make referral

To conduct needs assessment on physical, mental, and social health, and service needs, so as to provide support or make referrals to community resources or social services.

Improve home safety and environment

To carry out home improvement services for supporting the elders to age in place.

1 Priority will be given to elders who are financially deprived (e.g., recipients of Comprehensive Social Security Assistance / Old Age Living Allowance, etc.)

3For example: day care services, Community Care Service Voucher Scheme for the Elderly (“CCSV”), Integrated Home Care Services (“IHCS”), elderly centre or social activities, etc.
